Patient Charter: Our Commitment to Patients Experiencing Homelessness
The NHS is here to help improve your health and wellbeing and ensure that everyone has access to healthcare, regardless of their housing situation.
At Rothschild House Group, we understand that people experiencing homelessness may face additional challenges accessing healthcare and may have more complex physical and mental health needs.
This means that you can expect the following from our practice
- You do not need a fixed address, proof of address, identification or immigration documents to register with our practice.
- We will treat you with dignity, kindness and respect at all times.
- We understand that homelessness can affect people in different ways, including those sleeping rough, living in temporary accommodation, staying in hostels or shelters, or sofa surfing with friends or family.
- With your consent, we will ensure relevant information is recorded in your medical record to help us provide appropriate support and continuity of care.
- You can request to see the same GP where possible, helping us to build a supportive and consistent relationship with you.
- We will help make our services as accessible as possible and discuss the best ways to contact you regarding appointments, test results or ongoing care. We are also able to keep post for you until you are able to collect it from the Surgery.
- Where appropriate and with your consent, we can refer you to specialist support services, including mental health, substance misuse, social prescribing and housing support organisations.
- If you need support from other healthcare services, we will share relevant information appropriately so that you do not have to repeatedly explain your circumstances.
- We recognise that patients experiencing homelessness may have complex or long-term health conditions and we will work with you to help manage your healthcare needs safely and effectively. You will be offered double appointments wherever possible.
